Home Health Certified Occupational Therapist (COTA) - Jefferson Healthcare in the Victorian Seaport of Port Townsend, Washington is seeking a COTA to work part-time in our Home Health & Hospice Department. This position is 0.6 to 0.8 FTE, 24 to 32 hours per week, 8:30am to 5:00pm with rotating weekends and call. The Home Health & Hospice COTA treats patients referred for occupational therapy, communicates with occupational therapists for treatment plans and changes in the plan of care. Works with other health care professionals for optimal patient care. Completes documentation, billing and other paperwork per department policies. The patient load is a combination of home health, outpatients and inpatients. Monday - Friday, with occasional Saturday coverage as needed. Certification from Occupational Therapy Board. Washington State Licensure. Graduate of an approved/accredited Occupational Therapist Assistant associate degree program. Two years experience preferred. One year home health, outpatient, or skilled nursing facility desirable.

The Supervisor is a member of the interdisciplinary and leadership teams of Hospice is responsible for the implementation, delivery, coordination, and oversight of Hospice services delivered by self-managed care teams to insure that patient care services are delivered in a manner that is consistent with the philosophy and mission of Hospice and meets all regulatory requirements and standards. This position has direct supervision of Registered Nurses, Social Workers, Certified Nursing Assistants, Chaplain, Massage Therapist, and Intake Specialist. This is a first line management position requiring strong leadership and organizational skills. In addition, work situations requiring sensitivity to the values and expectations of medical staff, management, patients, and families.
Minimum Requirements:
- Minimum of two years Medicare Certified Hospice or Home Health experience preferred.
- Acute care nursing in surgical or medical Oncology, ICU, or ER preferred.
Education/Licenses/Certifications:
- Current WA RN license.
- Bachelor of Science in Nursing preferred.
- Certification as Hospice Nurse preferred.

Providence is calling a Quality Manager.
This position is responsible for leading work to achieve improvement in patient outcomes and key metrics prioritized in the quality plan to performance at or above the 90th percentile. Through the ongoing collection and analysis of data and in collaboration with Quality and local leadership the Quality Manager identifies areas with opportunities for improvement. When approved as priorities the Quality Manager facilitates Continuous Quality Improvement efforts to address opportunities by developing and deploying systems that will enhance results. This includes facilitating processes to assess, analyze, and trend data and conduct research and implement evidence based best practice to improve results. The Quality Manager is responsible for collaborating with leaders and engaging staff in evidence based best practice transitions to achieve quality outcome results. This position coordinates and provides oversight of systems that collect, track and trend patient, family and resident satisfaction and experience data. Responsible for influencing practice and system changes to improve the patient, family, resident experience to the 90th percentile. The position collaborates with local and Quality leaders and coordinates and facilitates process improvement efforts related to improving the patient/resident experience. Provides a proactive approach to meet patient, family, resident expectations by developing and deploying processes that are anticipatory and that interact with and engage patients, families, residents and staff. The position is responsible for training staff on Service Excellence skills and methods.

Position Specifics
This position is located at Providence Health & Services in either Everett, Olympia or Renton, Washington.
REQUIREMENTS
Education: Bachelor's degree, RN or other healthcare field, preferred.
Licenses/Certifications: Professional license where appropriate. Certification with Operational Excellence as a Change Facilitator or Green Belt preferred. Training may be obtained once employed by PSCS.
Experience: 2+ years healthcare quality improvement experience. Training in Operational Excellence or equivalent Quality Improvement process methodology. Experience in customer service and customer advocacy preferred. Project management or related management/facilitation experience preferred.
