Jefferson Healthcare in the Victorian Seaport of Port Townsend, Washington is seeking a Staff RN/Case Manger to work part-time in our Home Health & Hospice Department.   This position is 0.8 to 1.0 FTE, 32 to 40 hours per week, 8:30am to 5:00pm with rotating days.  The Home Health & Hospice RNs provide quality skilled nursing care to clients in their homes.  Requires WA State RN License, WA Driver’s license, current CPR.  One to two years acute care of home health/hospice experience.  One year home health and hospice experience preferred.  BSN preferred.

Home Health & Hospice Medical Social Worker (MSW)

Jefferson Healthcare in the Victorian Seaport of Port Townsend, Washington is seeking a Medical Social Worker to work part to full-time in our Home Health & Hospice Department.   This position is 0.8 to 1.0 FTE, 32 to 40 hours per week, 8:30am to 5:00pm with an occasional Saturday.  The Home Health & Hospice MSW provides short term psychosocial services to assist patients and their families to cope with personal, financial and environmental difficulties that arise from changes in their medical condition within the clinics, ER, the Hospital, and Home Health Program. Master’s degree(MSW)from a C.S.W.E. accredited school plus 2 years recent MSW experience required. Washington State Registered Counselors License.

Manager of Quality (RN) - Providence SoundHomeCare and Hospice

Primary responsibilities will include: being responsible for organizing and integrating functions of quality, health/safety and risk management, including performance standards and corrective action plans for the PSHCH. Report on results to appropriate oversight committees and quality boards. Assures that externally imposed regulatory or accreditation standards are met or exceeded. Reduce/eliminate risk exposure and/or liability. This position is clinically and operationally responsible for the Quality Coordinator, Performance Improvement/Education Coordinator. Requirements:

Licensure/Certifications: Current WA State professional licensing appropriate to education.

Requirements:
Education: Graduate with from an accredited school of nursing or rehabilitation (Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist) required.Experience: Prefer two years recent Home Health experience. Jefferson Healthcare in the beautiful Victorian Seaport of Port Townsend, Washington is seeking a Physical Therapist to work part to full-time in our Home Health & Hospice Department.   This position is 0.8 to 1.0 FTE, 32-40 hours per week, Monday through Friday 8:30am to 5:00pm with occasional Saturday.  The Home Health & Hospice PTs provide evaluation and treatment of physical therapy patients in their homes for Home Health & Hospice.  Licensed to practice physical therapy in Washington State. At least 2 years of PT experience required. Home Health experience preferred.  Requires WA Driver’s license, current BLS CPR.
